STOP USE AND RECALL/REPLACE OR REMEDIATE – IMMEDIATE ACTION REQUIRED

As part of 3M’s ongoing commitment to delivering high quality products, we are notifying our customers of the following information related to select 3M™ DBI-SALA® Sealed-Blok™ Self-Retracting Lifelines (SRLs). 3M Personal Safety Division has identified an aging effect that may impact the performance of the 3M DBI-SALA® Sealed-Blok™ SRLs produced from January 2022 to October 2024.

Additionally, any devices repaired by 3M, or an authorized service center (ASC), during this timeframe may also be affected. Through laboratory testing, 3M has determined that in the event of a fall, these devices may not lock up as designed which could expose the worker to serious injury or death. There have been no reports of injuries associated with this issue. Affected sizes are all variants of the 85 ft (~26M), 130 ft (~40M), and 55 ft (~17M) self-retracting lifelines, see appendix for the full list of models impacted by this stop use notice.

Due to this, we are sending out this Stop Use and Recall / Replace or Remediate Notice, so the select 3M DBI-SALA® Sealed-Blok™ Self-Retracting Lifelines are removed from service immediately. 3M is working towards having replacement product and remediation solutions available soon. Failure to read and follow all instructions associated with this Notice may result in serious injury or death of a user. Instructions for appropriate next steps are below.

Users/Owners:

Immediately stop using your Sealed-Blok™. To identify if your product is affected, follow the steps listed below:

Step 1: Locate the model number on the metal identification label (see Photo 1). If your device model number is listed in Appendix B, proceed to step 2. If the model number is not included in Appendix B, it is not part of the notice and can be returned to service.

Step 2: Locate the manufacturing and service date records on the metal identification label. If the device was manufactured OR serviced from January 2022 to October 2024, go to step 3. If the device was not manufactured or serviced during the affected timeframe, it is not part of the notice and can be returned to service. (See Appendix A for remediation mark on label).

Step 3: To begin the process for addressing affected device(s), please go to www.3m.com/SealedBlokRecall and complete the intake form which will help you to determine what options are available for your situation. Not all replacement or remediation options are immediately available at the time of this communication.

RETURN NOTICE – IMMEDIATE ACTION REQUIRED

If you are a customer that has purchased a Honeywell Miller Falcon+ SRL, the device is subject to an immediate Product Stop Use & Return Notice (see table below listing impacted SKUs). Honeywell has identified an anomaly related to the gauge of cable crimping on the SKUs below manufactured from April 4, 2023 through December 19, 2023. This anomaly could prevent the product from properly supporting weight per specifications.

Please review your Miller Falcon + SRL inventory immediately (see table with SKUs on the left), especially any units already in the field. If any match the SKU and part/model number as outlined above and have a manufacture date during the range noted above, immediately remove those units from service and/or quarantine them.

If you have purchased the unit(s) from a vendor other than 3M, please contact the manufacturer directly at:

3M’s Customer Service team will initiate a Return Material Authorization (RMA) to our Authorized Service Center.

For any Falcon+ SRL units with a date of manufacture on or after Dec 20, 2023, no action is required.

Overview: 

3M Fall Protection has identified a limited number of Protecta Harnesses where the harness webbing is not routed through the adjuster. This condition results in the adjuster not being able to be adjusted or does not lock in place after adjustments have been made. The assembly error is believed to affect only a limited number of units and has been corrected.

There have been no accidents or injuries related to this issue. This manufacturing issue will result in the harness not performing properly in the event of a fall, which could result in severe injury or death. Due to this, we are sending out an inspection notice so this issue can be detected by inspecting the harness as shown below.

End Users: Please follow the steps listed below:

Step 1: Remove product from service until inspection is performed.

Step 2: Locate the label on the harness to determine the model number and if it was manufactured between October 1, 2021 and November 1, 2022. If the harness is an impacted model and was manufactured in the affected date range, please continue to Step 3.

Step 3: Once you have confirmed the part number & affected date, inspect the torso adjusters to ensure the webbing is properly routed through the adjuster. If the harness passes the inspection, it is acceptable for use. If the harness does not pass inspection, contact our Customer Service Department to arrange to have the harness replaced, as per our standard warranty.

3M™ Fall Protection has identified a potential manufacturing issue with a limited number of 3M™ Protecta® Rebel Self-Retracting Lifelines (with galvanized or stainless-steel lifelines) produced between October 14, 2019 and February 25, 2020. There have been no reports of injuries or accidents associated with this issue. This manufacturing issue could result in the SRL not engaging properly but can be easily detected through the pre-use inspection as specified in the Protecta® Rebel Instruction for Use (IFU) document.

End Users: Please follow the steps listed below:

Step 1: Locate the label on the Rebel SRL to identify the manufactured dated (see picture to the right). If the SRL has a manufacture date of 19/10 (October 2019) through to the end of 20/02 (February 2020), continue to step 2. (Please note that regardless of the manufacture date, all SRLs should be inspected prior to every use and by a competent person annually as per the IFU).

Step 2: Pull the lifeline quickly to ensure that the SRL locks up. As per the IFU “Ensure the device locks up when the lifeline is jerked sharply. Lockup should be positive with no slipping.” It the SRL locks up properly and passes all other aspects of the pre-use inspection as defined in the IFU, the SRL is acceptable for use. (For a full listing of inspection criteria please refer to the IFU for your respective regions which can be found at www.ProtectaRebelInspect.com). If you find that your SRL does not lock up, take the Protecta® Rebel SRL out of service immediately. PRODUCT ADVISORY

As part of 3M Fall Protection’s on-going commitment to delivering high quality safety equipment, we are notifying our customers of the following information related to the 3M™ PROTECTA® Climbing Harnesses with Sternal D-Rings. This Product Advisory is to notify end users of the change in the compliance rating on a limited group (see table below) of 3M™ PROTECTA® Climbing Harnesses from ANSI Z359.11 to OSHA 1926.502 complaint.

This is not a recall and the 3M™ PROTECTA® Climbing Harnesses with Sternal D-Rings remain safe for use.

We are not aware of any accidents or injuries related to the change mentioned above. We have not received any customer complaints regarding this issue, and it was identified through internal testing.

Although products produced from February 2018 to May 2019 are labeled as meeting applicable ANSI standards, during internal testing it was determined that the vertical d-ring may not meet the dynamic requirements of ANSI 2359.11-2014 for the dynamic drop but does meet the requirements of OSHA 1926.502. Product produced after June 2019 are properly marked as being OSHA 1926.502 compliant.

PRODUCT ADVISORY

As part of 3M Fall Protection’s ongoing commitment to delivering high quality safety equipment, we are notifying our customers of the following information related to the 3M™ DBI-SALA® EZ-Stop™ Shock Absorbing Lanyard portfolio.

With respect to the voluntary ANSI/ASSE Z359.13-2013 standard, recent 3M testing produced results marginally (~2-4%) above the maximum requirement for testing in ambient, dry conditions.1 We believe this is the result of a raw material issue, which we are currently investigating (with in-scope product shipments currently on hold pending completion of that work). Product shipments will resume as soon as products are requalified to meet applicable requirements. We have not received any complaints from our customers related to this issue.

Because the EZ-Stop Lanyard portfolio products are safe to use and continue to meet all applicable OSHA requirements as well as remain well below the ANSI requirements for testing in hot, cold, or wet conditions, these products can remain in service.

All recipients of this mandatory stop use notice must read, understand and follow all instructions. Failure to do so may result in serious injury or death.

At Honeywell, our mission is to be the global leader in personal protective equipment with innovative solutions that protect and save lives. In line with our commitment to our mission statement, we are writing to inform you of potentially non-conforming brake components in certain models of Miller MightEvac and MightyLite Self-Retracting Lifelines (SRLs) ranges manufactured within a specific timeframe. While there have been NO reported incidents due to this nonconformity, continued use of the product could result in serious injury or death.

This notice only affects the models listed in Table 1 (in Notice), with identified manufacturing dates or re-certification dates between October 27, 2016 and October 6, 2018. Models outside of this date range, including new units, are not affected by this notice.

We have already determined a solution for the potential non-conforming brake components for affected models of Miller MightEvac and MightyLite Self-Retracting Lifelines (SRLs), which will require replacement of the brake component at our repair center. We apologize for the inconvenience this may cause and potential business disruption. We urge you to immediately discontinue use of affected units to avoid safety risks. Our team is working hard to streamline the return and repair process. Replacement of the brake component will be free of charge and at no additional cost to you. We will provide you with more details on the return and replacement process.

3M Fall Protection has determined that the 3M™ PROTECTA® Cobra Mobile/Manual Rope Grab AC202D is incorrectly stamped for use with 1/2 to 3/4 inch diameter rope on the exterior body of the unit. This Cobra Rope Grab is certified for use ONLY with 5/8 inch diameter polyester or polypropylene rope. All marketing information and the Instructions for Use (IFU) for this rope grab correctly identify the size of rope to be used as 5/8 inch polyester/polypropylene rope.

In the event of a fall from height, a Cobra Rope Grab used with 1/2 inch diameter rope may not arrest the fall and could result in serious injury or death to the worker. To remedy this situation, 3M is launching a Stop Use & Product Recall to replace all AC202D and related Cobra Rope Grabs that are stamped 1/2″—3/4″ Dia.

Users/Owners: Immediately stop using and inspect your Cobra Rope Grab as described below:

  • Step 1: Verify the rope diameter is stamped 1/2″—3/4″ Dia on the front side of the unit.
  • Step 2: Verify the rope grab is stamped PROTECTA COBRA on the front side of the unit.
  • Note: If the unit is stamped 5/8 inch diameter OR if the unit has a white label showing 5/8 inch diameter, then the unit is safe to return to normal use (see image on right for correct 5/8” diameter marking).

3M Fall Protection announces an immediate stop use and product recall of the 3M™ DBI-SALA® Nano-Lok™ edge and Wrap Back Twin-Leg Self Retracting Lifelines. The twin-leg Nano-Lok edge is part of a personal fall protection system and connects two self-retracting lifelines/devices (SRL’s/SRD’s) directly under the dorsal d-ring of a worker’s harness. It is intended to be anchored at foot-level, and is designed for sharp edge applications. The twin-leg Wrap Back Nano-Lok is intended for wrapping around an anchor and incorporates a similar energy absorber. 3M has determined that in the event of a fall and under certain conditions, the energy absorber may not properly deploy which could expose the worker to serious injury or death. Although there have been no reports of accidents or injuries associated with this issue, these products must be removed from service immediately.

To address this situation in the interests of worker safety, 3M is launching a global stop use of the Nano-Lok edge and Wrap Back Twin-Leg SRL units and is recalling all these units to be repaired or replaced as soon as a solution is identified, tested and certified. In the interim, and until a repair or replacement is available, end-users may elect to receive cash for their returned units as described below. This stop use and recall affects all versions of the Nano-Lok edge and Wrap Back Twin-Leg SRL’s since first introduced in 2013.

Users/Owners: Immediately stop using your Nano-Lok edge and Wrap Back Twin-Leg SRL and take these units out of service. Next, go to www.NanoLokEdgeRecall.com and follow the instructions on how to return your unit. As soon as a product solution is available, 3M will either repair or replace your unit and return it to you at 3M’s expense. Alternatively, and until a product solution is available, you can elect to receive cash in the amount of $200 USD when you return your unit. As an interim solution for applications requiring 100% continuous connection in leading edge applications with sufficient fall clearance, 3M recommends the DBI-SALA® EZ-Stop Leading Edge 100% Tie-Off Cable Shock Absorbing Lanyard. 3M Fall Protection has received a series of inquiries regarding the use of dorsal D-ring extenders with Leading Edge SRL’s in environments where sharp edges are present. Examples of 3M dorsal D-ring extenders are shown below, but the concern applies to all dorsal D-ring extenders, regardless of the supplier/manufacturer.

Testing shows that, when dorsal D-ring extenders are used together with Leading Edge SRL’s in a leading/sharp edge application, it is possible for the lanyard, subsystem, or other system components (such as the SRL shock-pack/energy absorber) to come into contact with, and be severed by, an unprotected sharp edge.

3M’s user instructions for D-ring extenders specifically direct users to avoid working where the lanyard, subsystem, or other system components will be in contact with, or abrade against unprotected sharp edges. If working with this equipment near sharp edges is unavoidable, then protection against the lanyard, subsystem, or other system components being abraded or severed must be provided by using a heavy pad or other means over the exposed sharp edge. The dorsal D-ring extender is approved for use in applications where the anchor is overhead and exposure to a sharp edge is not possible. 3M Fall Protection (formerly Capital Safety) has received field reports of broken end-posts (also known as Leap Energy Absorbing Posts, or “LEAP posts”) used in roof-mounted EvolutionTM Horizontal Lifeline Systems. There have been no reports of fall-related injuries associated with this condition.

To remedy this situation, 3M is launching a global Stop Use – Field Service Action to replace all roof mounted end-posts used in these systems between 2005 and 2015 (at which time the Evolution HLL system was discontinued).

A similar updated horizontal lifeline system is the Fixed Roof Anchors system.

3M™ Counterweight Anchor using an Evolution LEAP Post

3M Fall Protection (formerly Capital Safety) has received field reports of broken end-posts (also known as Lead Energy Absorbing Posts, or “LEAP posts”) used in roof-mounted EvolutionTM Horizontal Lifeline Systems. 3M has launched a global Stop Use – Field Service Action to address this situation. The same Evolution end-posts are also used in Freestanding Counterweight Anchor systems. There have been no reports of fall-related injuries associated with this condition.

To address the Freestanding Counterweight Anchor situation, 3M is launching a global Stop Use – Field Service Action to retrieve and replace all end-posts used in these Anchors sold between 2005 and 2015 (at which time the Evolution end-post was discontinued and replaced with a Roof Safe end-post).

PRODUCT RECALL/REPLACEMENT NOTICE – IMMEDIATE ACTION REQUIRED!

3M has issued a safety recall notice regarding the original Lad-Saf™ sleeve. It has been discovered there could be potential misuse scenarios that could result in serious injury or death. It has been replaced by a completely redesigned next generation Lad-Saf sleeve X3.

This product was sold under the Capital Safety and DBI Sala brand names and is used for ladder fall protection.
